AccuRx requests can be submitted by the patient or by a third party on behalf of the patient. The form will prompt you to add the details of the patient and also the details of the person filling the form out, if appropriate. Please ensure the information provided is put in the correct sections as the information goes straight to the patient record when the clinician is triaging the request.
